Elevate - Middle School Day Camp

How To Register

1. Read all the information about Elevate. Call 303-499-3553 if you have further questions or just want to talk to one of the staff.
2. Download and print the registration form, or call 303-499-3553, and we will send you a hard copy of the registration packet.
3. Fill out the registration form completely. Be sure to read and sign the "Assumption of Risk and Liability" form.
4. Send a deposit check of $75 (non-refundable registration fee), or pay the full amount of $125. Make the check payable to Elevate. If a deposit only is made, the balance is due the morning of the first day of camp.
5. Send the completed form and check to: Elevate, 7690 Baseline Rd., Boulder, CO 80303.
6. We will send you a confirmation of registration.
